Discovery Call Application

Please take a few minutes to answer honestly. Your answers help us make the most of our time together on your call.

What's going on that made you fill out this form today?

On a scale of 1–10, how would you rate your energy on an average day? (1 = running on fumes, 10 = fully charged)

When you picture ‘burnout,’ does that word describe you? Why or why not?

What does your body feel like most days — sleep, digestion, tension, pain, illness? Be specific.

Who in your life is getting the tired, short-tempered, or checked-out version of you right now? What has that cost that relationship?

What have you already missed, postponed, or quietly given up on this year because you didn't have the capacity for it?

If a close friend described how you're living right now, what would they say?

What part of yourself — your patience, your creativity, your sense of humor, your ambition — do you feel like you've lost touch with?

How long has it felt like this — not the worst week, but this general state?

What's changed about your health, mood, or relationships over that time that you've mostly kept to yourself?

If you're still living exactly like this one year from now, what does that cost you — physically, in your relationships, in your career?

What are you most afraid will happen if you keep pushing through this the way you have been?

Is there a version of your future self — a parent, a partner, a professional — that you're worried you won't get to become if this continues?

What have you already tried to fix this — more sleep, supplements, therapy, vacations, willpower? What actually happened?

Why do you think none of that has stuck long-term?

On a scale of 1–10, how ready are you to actually commit time and energy to changing this in the next 90 days?

What would it mean to you — and to the people around you — if you felt like yourself again in 12 weeks?
